All payments are due at the time of service. Our office does not deal directly with insurance companies. For your convenience, we provide insurance superbill statements. Make sure to request a Medical Necessity Letter and have it signed by the office, if you believe your insurance provider will ask for one. Click HERE for a copy of the form.
