Before your first appointment
New patients, in order to save you time on your first visit we have compiled a packet of patient information. Please click the link below to download the packet of PDF files. You will find three forms that will need to be completed and brought with you on the day of your appointment. This is described on the first page of the packet. Also, please bring your most current lab test results, CT and MRI reports, prescription medications, and all nutritional supplements that you take to the initial visit.
